Do any of you know how to lay hardwood floors?

Before we (hopefully) move into our new house, one of the things we're going to do is to replace the worn our carpeting in the house with real (solid) hardwood floors. Both my wife and I are pretty handy and the woodworking/fitting aspects of the hardwood install are well within our capabilities. I've read a number of articles, watched quite a few videos, and have a very good understanding of how to lay the "middle" 95% of the flooring. I also understand how to bring it up to a wall that'll have baseboard trim while allowing for wood movement. There's three sticking points I haven't seen anyone talk about yet, though.

1) What do you do when you bring the wood flooring up against some other kind of flooring? For instance, there will be tile in the kitchen and the bathroom. The bathroom tile will meet the hardwood at a 45* angle. The kitchen tile will be a 90* butted up against the end grain. How do I finish these in a way that doesn't leave a visual gap but does allow for wood movement?

2) What do you do where the hardwood meets the edge of the floor? The upper floor is lofted with a railing at one end and I'll need probably a quarter-round profile on the edge of something to meet that. The upper floor also (obviously) has stairs and I assume some sort of half-round bullnose to wrap over the edge.

3) What do you do when the long grain runs parallel to a door that doesn't have a sill that overhangs the flooring?

So, if any of you can answer these questions or, better yet, direct me to books or videos that talk about how to do these things, I'd be very thankful!
